Every section, every question, its kind and whether it is required, with the conditional ones showing the answer that opens them.
No, and that is deliberate. A set is copied onto the posting when you attach it, so a posting that is already taking applications keeps asking exactly what it asked on day one. Everyone who applied answered the same form.
Yes, at either level. A whole section can appear only when an earlier answer calls for it, and so can one question inside a section, so a certificate number is asked only of the people who said they hold a ticket.
Yes. First name, last name and email are standard on every form and cannot be removed. Everything else is yours to decide, including whether to ask for a phone number at all.
No. Nothing rules somebody out on the strength of one answer, and no single answer is scored. The conditions decide what somebody is asked, not what happens to them. Reading the application is a person’s job, with an assist if you want one.
Every submission carries two quiet checks: a hidden field that only an automated script would fill in, and how long the form actually took. A form completed in two seconds looks different from one completed in six minutes, and the difference is recorded rather than guessed at later.
